As a science-based non-profit organization, public trust in the integrity and independence of the American Heart Association’s scientific review and decision-making processes as well as American Heart Association’s adherence to high standards for the conduct of its charitable activities is essential. To protect both American Heart Association and you, the Conflict of Interest Policy requires disclosure of relationships by American Heart Association Representatives such as you in your current role(s), and resolution of any conflicts by American Heart Association to ensure that actions taken are in the best interest of American Heart Association. This protects American Heart Association’s and American Heart Association Representatives’ decision-making from the appearance of bias or improper influence by individual personal or business interests, family or close associates in American Heart Association.
As an American Heart Association Representative we ask that you disclose ALL financial and non-financial relationships for the past 12 month period in which you or your immediate family members have direct or indirect monetary interests, decision making responsibilities, or positions of influence, as described in the table below. If you have more than one role, please follow the instructions that require the most disclosure.

The American Heart Association will evaluate these relationships to determine if any actual or perceived conflicts of interest may exist based on your position(s) and scope of decision making authority, the substantiality of the relationships, the pervasiveness of the conflict and whether additional measures are needed to protect the integrity and reputation of you, as the American Heart Association Representative, and the American Heart Association.
